Please note, simply filling out the attestation form as a parent and participant, is NOT acceptable.
Exemptions to the vaccine policy are as follows:
Medical exemption - see the details in the policy. This exemption must be obtained by a licensed, practicing MD in Ontario, in good standing with the College of Physicians and Surgeons of Ontario, and must be submitted to the [email protected] email address 24 hours prior to the participant's first tryout FOR APPROVAL. Do not present at tryouts on Friday without an approval email from the COVID Team, expecting to participate.
The documentation form must be on the physician's letterhead and contain the name and DOB of the patient, the name and CPSO number of the physician, and physician signature. That must be submitted to the [email protected] email address FOR APPROVAL. If approved, you may attend tryouts, with proof of a negative COVID-19 Antigen Screen, at the player's expense, which is good for 72 hours only. A repeat test is required for continued participation in STMHA sanctioned events.
MEDICAL_EXEMPTION_TO_DECLINE_THE_COVID
Request for Creed/ Religion Accommodation - Please make sure to see page 2 of the exemption request form. The request must be complete, including the answers to the 5 questions on page 2. Once complete, the request form MUST BE NOTARIZED by a notary in good standing with the Law Society of Ontario. Once the form has been completed by the participant, and has been notarized, it is then submitted to the [email protected] email address FOR APPROVAL. Do not present on Friday without an approval email from the COVID Team, expecting to participate.
