INSTRUCTIONS: 1) Cost of tryouts is $85 and is due at the first tryout or in advance in the STMHA office. 2) Please bring all required forms to the first tryout or to the office in advance (NRP Passports, copy of Birth Certificate if new). 3) Tryout fee is non-refundable. 4) One parent must complete Respect in Sport Program before player can be rostered. 5) Please complete all sections.
