To be completed by Travel Team Coaches seeking permission to attend a tournament.

Payment Request Information

please complete the following section if you are requesting payment for tournament fees.

I understand that no team may participate in a tournament while participating in OMHA Playdowns or Alliance League Playoffs.  As a general rule tournament requests from mid January to end of March will be declined for this reason.

Alliance teams are limited to 4 tournaments during the Alliance regular season.

As coach/manager of the above team I agree that any and all money sent out by STMHA to pay a tournament entry fee (as a result of this request) will be fully reimbursed by my team or me personally by November 30 of this season.

Lastly, I acknowledge that STMHA will only issue payment for tournament registrations directly to the host minor hockey association OR to the team for whom the request is being made (i.e. St. Thomas 2010 Jr Stars).

STMHA is able to issue E - Transfers to host centres on behalf of our teams.
