Late Registration Fees now in effect
FEES & PAYMENT INFORMATION:
2011-2012 Fees:
Initiation Program (Born in 2005-2007): $450 (includes late fee)
General (2004-1992): $500 (includes late fee)
Lottery Fee*: $25
Travel Tryout Fee**: $55
Body Checking Clinic***: $35
Late Fee - After July 1st: $50
* One Fee Per Family
** Must be paid by all players wishing to tryout for ANY Travel Team (includes Tyke & Novice Select)
Players trying out for a Travel Team must be registered by AUGUST 1st
*** Recommended for Pee Wee aged players wishing to tryout for any Pee Wee Travel Team.

NEW TO ST. THOMAS MINOR HOCKEY?
NEW PLAYERS must supply a copy of their Birth Certificate
If you have previously played hockey and have moved from another Centre or Town
a Residential Transfer Form and it's supporting documents must be supplied at time of registration.
A Residential Transfer form is not required for moves within the City of St. Thomas.

WHERE TO REGISTER:
In person at the St. Thomas Minor Hockey Office (upstairs in the Timken Centre
on the STMHC Rink). Office hours are Monday to Friday 10-3.
Or
In person at one of our Registration Nights:
Tuesday, May 3rd - 6pm to 9pm at the St. Thomas Timken Comminity Centre
Saturday, June 11th - 10am to 1pm at the St. Thomas Timken Community Centre
Tuesday, June 28th - 6pm to 9pm at the St. Thomas Timken Community Centre

PERMISSION TO SKATE FORMS:
Players wishing to tryout for AAA will require a Permission to Skate Form from St. Thomas Minor Hockey.
Please contact St. Thomas Minor Hockey [email protected] to obtain a form.
Please note Permission to Skate Forms will not be issued until the player is registered with STMHA.
